At SWIFT, our mission is clear: to support individuals and families in finding stable, permanent housing and achieving lasting stability. Our dedicated team offers a variety of programs designed to meet diverse needs and provide comprehensive support. Explore how we can help you or someone you know find their path to a better future.

OUR PROGRAMS

Permanent Supportive Housing (PSH)

Permanent Supportive Housing offers a community-based solution with no designated length of stay. We provide rental assistance and ongoing support to individuals and families with disabilities, address both mental health and physical needs. Our program ensures that participants receive:

• Monthly Counseling Sessions: Support services staff conduct regular counseling to help maintain stable housing and address any emerging needs.

• Comprehensive Support: Our focus is on ensuring long-term stability and integration into the community.

We collaborate with the Rochester Housing Authority, offering two distinct PSH options:

• Community-Based Housing: Find and secure suitable housing within the community.

• Project-Based Housing at Eastman Commons: Ideal for single participants seeking housing in a supportive environment.

Rapid Rehousing (RRH)

Rapid Rehousing is designed to help homeless families and individuals, with or without disabilities, quickly move to permanent housing and achieve stability. Key features include:

• Short-term (up to 3 months) and Medium-term (3 to 24 months) Rental Assistance: Financial support for up to 24 months for those with or without disabilities, move as quickly as possible to permanent housing and achieve stability in that housing.

• Monthly Case Management: Personalized support to address challenges and ensure a smooth transition to stable housing.

• Move-in Assistance: Help with the costs associated with moving into a new home.

Rapid Rehousing for Domestic Violence Survivors (Tapestry)

Tapestry offers specialized support for survivors of domestic violence, providing all the benefits of our Rapid Rehousing program with additional tailored assistance:

• Collaboration with Willow and Legal Aid: Enhanced support through partnerships with specialized organizations.

• Comprehensive Support Services: Ongoing assistance from a network of providers to ensure safety and stability.

• Monthly Case Management: Dedicated support to help navigate challenges and secure long-term housing.

ACCESSING OUR PROGRAMS

To access any of our programs, individuals must be referred through the community-wide Prioritization List. All clients must be experiencing homelessness and working with an outreach worker.
