CARE MANAGEMENT SERVICES

Health Homes is not a physical place.  It is a group of health care and service providers working together to make sure you get the care and services you need to stay healthy. 

Is Health Homes Right for You? 

• You are an adult 18+ 
  You receive Medicaid 
  You have qualifying chronic health conditions 
  You are interested in having a Care Manager help you get the 
  care and services you need to stay healthy 

What Can You Expect from Health Homes? 

• Coordinated communication with your care team 

• An advocate who works to get you the care you need 

• Help navigating the complex health care system 

• Resources for basic needs 

• Education about keeping yourself healthy
